Read the Service Contract and the HIPPA Privacy Policy.

 

Print and complete the PCP Release Form, Contact Information Form, and the Acknowledgment and Agreement Form, and bring to your first session. If you do not have a PCP or do not want us to notify them of your treatment, you may skip that form.
